EMDR Reprocessing Session Log
About this Resource
I took the first part of EMDR training almost immediately after getting my counseling license. Although I now have all the steps and the “script” stored firmly in my memory, as a new EMDR practitioner (and a new counselor), it all felt like a lot to remember. I now keep some minimal notes on target memories on a notepad during sessions. But, as I was learning, it was helpful for me to have a template to fill out for each session. The resource I am sharing here is a version of the EMDR reprocessing session log I created and used for the first 6 months to a year of practicing EMDR. I have added some form fields to the PDF so that you can add some of the resources (i.e., grounding skills) that you commonly use with clients before you print copies for yourself.
Tips for Using this Form
A few abbreviations are used in this form to save space. C is short for counselor or clinician. Cl is used for client. Tapping (c admin.) refers to clinician-administered tapping.
The Phase 8 section is there to allow you to note whether the previous target maintained ecological 0/7 and therefore did not need continued reprocessing. You also have the option to mark continued reprocessing. I would often mark this box and simply write “same” for memory, image, cognitions, etc. if they had not changed compared to the previous session of reprocessing. I also included the light gray text at the end of the positive cognition and the emotions lines to remind me when to prompt the client for the VoC and SUD scores. The middle gray box in the SUD and VoC tracker grid is for intermediary ratings, to help you track progress during the session. This is not strictly necessary but can be helpful if the scores go up before they start decreasing as sometimes happens.
